Field Trip Liability Waiver

ACCIDENT WAIVER & RELEASE OF LIABILITY FOR PARTICIPATION IN ACTIVITIES AT STONE VIEW FARM
This is an ACCIDENT WAIVER AND RELEASE OF LIABILITY for Stone View Farm. Please read this waiver and submit the form before participating in activities at Stone View Farm.

I. I understand that by signing this Waiver, I assume all risk for myself and those for whom I am responsible, including my children, students, scouts, and other members of my party, participating in the activities at Stone View Farm. I acknowledge that activities at a farm pose potential risk of personal injury and that I willingly undertake and assume this risk for myself and those for whom I am responsible. II. I certify that myself and those for whom I am responsible are in sufficient shape and fitness to participate in activities at Stone View Farm, and that we have not been advised to not participate by a qualified medical professional. I have not been advised of any reason reason which would limit my or those for whom I am responsible from participating in activities at Stone View Farm. III. On behalf of myself and those for whom I am responsible, I further waive and release the owners and managers of Stone View Farm and promoters of the activities, and their directors, board members, officers, employees, volunteers, interns, agents, representatives, or assigns, as well as the activity sponsors, from any and all liability, including, but not limited to, liability arising from negligence or fault of the entities or persons for any injury or disability which may occur as a result of my or my child’s participation in the above activity. I am assuming all risks on behalf of myself and my child that may arise from negligence or carelessness on the part of any of the persons or entities being released, as well as from defective equipment, real property or personal property that is owned, maintained or controlled by the above persons. III. In consideration of my application and permitting me to participate in these activities, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, Stone View Farm and/or their directors, officers, employees, volunteers, interns, representatives, and agents, and the activity holders, sponsors, and volunteers; (B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned here in from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. IV. I acknowledge that Stone View Farm and their officers, volunteers, interns, representatives, and agents are NOT responsible for errors, omissions, acts, or failures to act or any party or entity conducting a specific activity on their behalf. V. I acknowledge that my participation may require physical and mental exertion and carry with it the potential for heat related illness or death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, equipment, vehicular traffic, lack of hydration, and actions of other people, including, but not limited to, participants, volunteers, interns, monitors, and/or producers of the activities. VI. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. VII. I understand that this Accident Waiver and Release of Liability Contract shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. VIII. I certify that I have read this document and I fully understand its contents. I am aware that this is a release and indemnification of liability for myself and those under my responsibility, and I sign it of my own free will.
